Concussion Management

1) LSA Informed Consent about Concussions – for parents/players – mandatory

2) LSA Concussion Notification Form – to be used at your tournaments/practices/games when a possible concussion is suspected (not required by law; provided for your information & use if you wish to document incidents); LSA will be using this form at all State tournaments

3) Concussion Procedure and Protocol to go with the above notification form; again, this is not required by law but will be followed at all LSA state tournaments

4) Act 314 – Louisiana Concussion Act for your reference
